Fragility fractures are a common clinical problem seen in aging adults and in specific populations with accelerated bone loss, such as individuals with spinal cord injury and postmenopausal women. Because bone adapts its shape and structure to best resist the mechanical forces to which it is typically exposed, alterations in habitual loading can cause measurable changes in bone strength. In our laboratory we use quantitative image analysis and computational (finite element) models based on computed tomography (CT) data sets to assess changes in bone structure and strength due to alterations in the mechanical loading environment. I will present examples of how we use patient-specific finite element models to understand the mechanisms of bone adaptation in healthy adults, and how these same techniques can be used to aid in clinical decision making and to assess the success of clinical treatments to preserve and improve bone health.
